All those are more philosophical problems than technical. Once you've decided what resources your target font(s) provide and what you'll use to replace the various punctuation marks, substitution is easy, easier than dealing with the digraphs. For anything unanticipated, you can have your algorithm flag it in some way, and then you can run it again. This wouldn't need to be real-time, if you're using the output for printing.
