"Penultimate" means "last but one," or "second-to-last." When you use 
{-ghach}, you generally need to put another suffix on the verb first; a 
bare verb plus {-ghach}, while intelligible, is considered wrong.

So for instance, in {qeqnISghach} "need to train" (a noun), the 
penultimate, or second-to-last, suffix is {-nIS} "need." In 
{qeqbe'ghach} "lack of training," {-be'} is the penultimate suffix.
