Had the same problem with a 3c595 card. Turned out that the case was causing the
card to pull slightly out of the
socket. I untightened the screw, reseated the card and the initialization went
just fine. Eventually had to replace the
case for one that did not pull on the cards. If this is not your problem, make
sure you have connectivity from your
card to the destination or hub (ensure it did not get unplugged or the cable was
not cut), or try a different card.  If
you are using an ISA card, such as a ne2000 compat. WITH switches/jumpers, make
sure a switch setting did not
change or a jumper fall off. If you are using coax, make sure the terminators
did not come loose or fall off.
