4.0 upgrade fails

I currently have an i586 3.0.3 system which has had (I think) all its
updates applied, including the 2.0.0 kernel.  I am now attempting to
upgrade to 4.0 (by FTP using the most recent boot and supp images, in
case it matters).

The procedure fails after having retrieved uglist with a
	Cannot open /mnt/var/lib/rpm/packages.rpm
(or similar) message in the install log.

The current rpm I have installed, 2.0.11, is quite happy to work with
this file (which does exist), including from the installation shell on
VC2 (after suitable system massaging).

Any advice would be appreciated.
