apache / httpd / server side include not working

I think I have turned on everything correctly, but my
server side includes don't work. Any suggestions?
Here is what I did...
in access.conf: made sure Options included 'Includes'
Options Indexes Includes ExecCGI
in srm.conf:
DirectoryIndex index.html index.shtml ...(and more stuff)...
AddIcon /icons/layout.gif .html .shtml .htm .pdf
# To use server-parsed HTML files
AddType text/html .shtml
AddHandler server-parsed .shtml
Anything else I am missing????
