Mount failure during installation

I keep running into mount failure after selecting install everything
during the install process.

I have a 75 Mhz Pentium.

With 2 Hard disks,  

C:  is ~500 MB with Win 95 installed.
D:  2.0GB recently installed in the computer.  I want to put Linux here.

1 CD ROM.

E:  Sound Blaster attached 4x CD ROM.

If I use extended partitions on during linux fdisk part of install,
I get Mount Failure after selecting install everything.

If I only use primary partitions, the install process works.

I verified that the D: drive is working by making sure that Win95 recognized
it before starting to install Linux.
