RH-4.0 installation can't find my cm206 CD drive

This is my first post to this list after a couple days' reading.
As noted, I am unable to install RH-4.0 from a purchased RH distribution.
When I try to tell the install program where to find my cm206/cm260 CD drive,
it just reports that it can't find such a beast in my system. Autoprobe
works just as well. Updated boot.img works just as well.
I have tried to build a kernel specifically for my CD and system, using
the 2.0.18 source tree from archives. It boots from HD, but I haven't been
able to install it successfully onto a boot.img. I have re-jumpered my
adapter to port 0x340, irq 11, matching the source tree in the 'live'
directory on my CDROM, but it is still not identified. (BTW, what happened
to the 'liveboot' file mentioned in the manual??? I can't 'find' it on
my CD. Nor could I find any info on such omissions in the Errata -- did
I just miss that?)
Parameters: 386dx40 w/387; ISA box with no secondary bus; 2 IDE drives
on one adapter; 8 Mb RAM; Phillips cm206 drive w/ cm260 adapter. Low-
rent, but works on my Slackware-derived 1.2.13. (Not a troll -- I don't
know if there are problems which arise from building for RH in a Slackware
directory structure. Certainly my partitioning is quite different from
the RH-4.0 manual's recommendations.) Currently all installed software
is a.out, but I have built an apparently usable boot set for 3.0.3, and I
could do a two- stage installation if that would help. I only have about
350-400 MBy HD space available for total Linux installation.
I would appreciate any suggestions, including fairly detailed instructions
for building an installation boot disk. (BTW2, I captured the ../trees
directory from our RedHat mirror -- also missing from the CD -- and can't
run mkboot2 due to inability of the script to find a free loopback. My
1.2.13 installation only provides two loopback paths, and the script won't
use them, even if I kill 'diald'. Is the number and detection of free
loopbacks a RedHat-ism, a 2.0-ism, or is my configuration an artifact of
Slackware's net setup?)
