I had a similar problem when I did a clean install of RH 4.0 onto a
system with an Adpatech 1542, /dev/sda1 as my main linux partition. I
fixed it in 2 steps:
1. Booted using an older (1.2.13 kernel) boot disk. Ignored any error
messages about missing modules, etc.
Went to /usr/src/linux and built a new kernel with built-in support for
the 1542 and a few other things I had to have. Installed it in /boot,
editied /etc/lilo.conf, reran lilo.
2. Rebooted with new kernel. Since /dev/loop now recognized, I can run
mkinitrd to get module support for the things I ocassionally want that I
didn't already build into the kernel. Reboot again to make sure it all
works.
Except for a strange crc error (that messes up the drive file system
check on boot), things now work just fine.
