It's likely that you did things in the wrong order; that's all I
can think of.  I've personally tested all the upgrades in the
errata step-by-step as listed, and never had any problems.  In
the next revision of the errata, we'll be expanding the "Upgrading
from Rembrandt" paragraph to explain that it covers more situations.

I assume you have fixed the problem now as well as found it; if not,
read that point in the errata (see the address in the footer) and
apply the simple fix.
