A reboot won't help any.

You need to upgrade pam (as you did) *and* run "rm /etc/.pwd.lock"

I'll have to have that explained clearly in the errata.  Sorry for
the confusion; I imagine it's my fault it wasn't clearly explained
in the errata in the first place.
