I've got the same modem in a Micron machine.  Early on it was a nightmare
to get it going.  Now it's much easier; you need to pick up the utility
isapnptools from the Red Hat site:

   http://www.redhat.com/linux-info/pnp/other.html

This will configure the modem (and other PnP devices, if desired)
appropriately.
