Ok, here's what I usually do:
1) I unpack the program I want to patch and install. In most cases, this is
a tarball, so I untar (gunzip, if necessary) it and usually end up with a
subdirectory containing all sources.
2) I copy the patch file into that directory.
3) if necessary, I gunzip that file (if the patch file ends in ".gz", that
is)
4) I use "patch -p1 < patchfile" to patch.
I have never patched the kernel (so far...), so I can't help you with your
particular case, but maybe the generic steps are already of some help.
