Actually, a PDF copy of the form is available on the National Website.
Just click on PUBLICATIONS, ALPHABETICAL LISTING, and it's the last item
listed:

Although it is fairly low key; i.e. we don't see a blazing announcement
front and center on the website (why would we want this to detract from
our Point of Light award, eh?). I think the announcements telling
volunteers to fill out this form will probably come via the RDs and SCs
themselves, through their own staff listservs and such,... Which is
definitely a better way of doing it because if they announced it on the
website, you'd probably get a bunch of pseudo-staff-alumni types
registering as volunteers that haven't been formally appointed to staff,
which is not what the NO wants to accomplish. So I would assume that
everyone that's on staff will probably be getting some type of email
shortly from their RD or SC asking them to fill out the form. But
obviously, before such announcements are made, they want to make sure
that the form is available on the website, so they don't make themselves
look stupid,... Until we see such an email, I wouldn't worry about
filling out the form,...
