Brothers,
I think there's some misunderstand as to how this form is to be used (and
the misunderstanding might be mine).
As *I* understand it, the forms were provided to the Chairs of National
Committees, RDs, and Section Chairs. I think then they were supposed to
send the forms to their volunteers (staff members, committee members, etc.)
I believe that was to be the way things were intended to happen, and
probably that's why the form itself wasn't posted on the web site.
And, truthfully, that's probably a more prudent way to do it than to just
make the form available.
