Addition of pthread_cancel to pthreads beta 4.1

I have added some functionality to the pthreads package which
people may find useful. Added functionality includes pthread_cancel(),
pthread_setcancel() and two additional functions [not sure whether
they are in the draft pthreads standard], pthread_suspend() and
pthread_restart(), which allow a thread to stop and restart other
threads.
I have tested these routines a little, but suspect that will be
many potential bugs lurkingi - this is an *ALPHA* release. The modified
pthreads beta 4.1 source tree will shortly be available by anonymous ftp
from ftp.ox.ac.uk
