Sun (who defined the PAM API) have not been very helpful with this.
Basically, for all the reasons that you have already discussed [in
summary, there is no standard] they decided to leave utmp maintenance
up to the applications.
We would all welcome a standard, it would be a simple thing to make
a session module that PAM aware applications could make use of.
If someone can come up with a coherent scheme, I'd be more than happy
to (help) implement it.
