My view of add-single-device is as follows:

   it should send a Bus Device Reset message to the device
   it should then wait a bit
   it should then do a complete initialization (inquiry, read capacity,
   everything applicable)

also, the sense handling routine should be modified to recognize
NOT READY, START UNIT REQUIRED and send the start unit.

Now, how easy is it, and how portable between SCSI HAs is it to
send Bus Device Reset messages ?
