Yes, this is documented to happen. If you forget to clean your drive
regularly (every 20 hours of use is a good compromise), the drive might
have its head clogged and may time-out when loading a cassette.

The cleaning tape must be changed every N use, N is indicated on the
cleaning tape (it's usually a bit more).

When you have replaced that drive, I suggest you verify all your
important backups, too. You could also look at the error counters.
