I went digging in the SCSI-II spec, and it looks like key 4 is "hardware
failure".  The "code 40" and "qualifier a0" are specific to the tape
drive.  Our documentation for another HP drive indicates this
combination is a diagnostic failure of whatever component a0 refers to.
