Problems with mke2fs and SCSI, 1.3.12+quotas

Hi,
I'm running a Fujitsu 230MB magneto-optical drive on a Buslogic 445C
controller. When running `mke2fs -v /dev/sda1', immediately after the
`filesystem accounting information:' I get:
Attempt to allocate device channel 0, target 0, lun 0
Kernel panic: No device found in allocate_device
No other information and the system dies.
Excerpt from my dmesg:

I tried the same command on /dev/fd0, no problems. Any assistance or a
starting point would be greatly appreciated. I'm running 1.3.12 and quotas,
and the error occurs consistently with my MO drive.
