I haven't used an Ultra2 card yet, much less an NCR8951U (I'd likely use a
Mylex), but this sounds like a very simple problem to me. The driver isn't
built for 40MHz. It's possibly getting the response too quickly, and
missing it. In the meantime, forego the 40MHz mode, and stick with 20MHz.
It works, so don't mess with it. ;)
