I need info about EVEREX-SCSI card, Help

I've a SCSI-card from Everex but i can't get it functioning with Linux
or with win '95 because I've got no software for this card.
Can anyone HELP me with info, DRIVERS or whatsoever?
What I can find from the card is the following:
everex IO power make in USA
EV-8116 Rev.C PWA-00706-
EPROM: 8116-C-19A ver 2.02
chips: AM5380PC(926CRA2) , ~ +B8948/DP8473N , FCB61C65L-70P
Connectors: J2 SCSI, J1 floppydisk, J3 external connector
PLEASE HELP!
