Redhat 4.2 installation can't mkfs -c Seagate disk

I am using an adaptec 1542cf scsi controller and have had no problems
using it with other scsi devices/disks. I am installing fresh on a
Seagate
Medalist Pro 2160N SCSI Hard Drive (2Gb). I partitioned it as:
900Mb DOS
40Mb  /
40Mb /var
947Mb /usr
130Mb swap

When I say check for bad blocks during the mkfs'es, the script always
hangs
with the scsi controller light on. 
When I say check for bad blocks during the mkswap, the script NEVER
hangs
with the scsi controller light on. 

I called Adaptec and they recommend I low-level format. I did the
low-level format followed by a verify media. Same problems.

I can get past the mkfs step by not "checking for bad blocks".
If I select install "Everything" (451Mb), the install hangs with the
scsi controller light on after ~280Mb.
If I select individual components to install ~150Mb, the install
successfully completes.



On a completely different machine (Gateway P5-100 with 1Gb IDE drive
and stripped down to 8Mb RAM). The install script recieves a signal
11 (segfault) when trying to install "Everything".
If I select individual components to install ~150Mb, the install
reports an ocurred while installing Bison and RPM but after clicking
OK, the script continues to run and completes successfully.

Am I having hardware or software problems?
