Did you try it the way I suggested?

	append="aha152x=0x340,11"

That's it.  Don't give it more than it needs.

Apparently you specified the argument for controller 0 and 1, in other
words, two.

Is this the same output as your showing us?  Please verify :)

Hmm. Reply with the ... you have listed there.  Possibly depmod didn't
run, and its running the module from the wrong directory.  Try deleting
your old modules from /lib/modules, and 'make modules_install' from our
linux src tree.

I know what its like to feel stranded.  I certainly don't mind posts like
this.  This is how bugs get found as well :)
