named sometimes rejects requests

I am running Slackware Linux 1.2.13, named 4.9.1.3 - locally as the
primary name server. The DNS lookups occassionally return an error.

The proxy-server usually saves a cached page in a directory name
such as www.fred.co.uk but occassionally it save it in a directory
based upon the target site IP number.

Similarly, every so often a web page request is rejected reporting
that "Proxy request failed: Host unknown or connection refused"
This happens when using the proxy server, a similar error occurs
when the proxy is not being used. The error is not repeatable,
subsequent requests work ok.

The response is reliably less than 1 second for the error to appear.
My interpretation is that the DNS returned an error - perhaps, it
was unable to get some resource for the lookup and rejected it.
Can someone suggest a fix, the cause or a test to try out? I am
quite prepared for the "Have you forgotten to ..." replies.

I will move to 2.0.Something when the PPP and TCP postings drop to a
less worrying level. But I would rather fix this first than upgrade
and find it is still there along with some other problems.

The log file for the last three weeks shows some errors. I have listed
their form and frequency. I know that the .53 is the nameserver port.
But I would like to know the reason for these messages. Is it a fault?

6 Malformed response from [<ip number>].53 (out of data in final pass)
25 Response from unexpected source [<ip number>].53
