A couple people have said that they've had the xscreensaver logout
button pop up immediately when they locked their screens.
I can't figure out how the code would be able to do that, but they
also can't figure out how to duplicate the problem, so rather than
trying to find a heisenbug, I've installed a workaround that should
prevent it from happening again.
