failed RZ23 disk on charon

charon, a DECstation 3100 server in W20-575A, had a failure of an RZ23
disk early  this afternoon. rzdisk -s gives the error "Unrecoverable
read error at (LBN = 8631)". fsck gave the initial error:

  RZ23 unit 3: harderr, SnsKey = MEDIUM ERROR, AddSnsCode = 0x11
  LogBlkAddr = 8631, Dev = rz3a, SectNum = 8631
  CANNOT READ: BLK 8624
  CONTINUE?

Upon continuing, there was a report of 11 additional unreadable
sectors, and fsck asked about whether to remove each file in
/etc/zoneinfo. We then deleted all the partitions on this disk from
/etc/fstab, and the system is currently running (although with very
little swap space) with its remaining disks.

We would be interested in obtaining a replacement for the RZ23 asap.

Incidentally, this disk was itself a replacement for one that failed
earlier this year (at the time, it was on a different server,
bloom-picayune). It was installed by DEC Field Service on January 4.
