It would help if the various notes were included with the sections
they apply to. In the printed form, it would also be good if they
were on the pages where they were referenced, instead of pushed to the
end, but given the apparent html "master" form, that's probably way
too much to hope for.
And what's the "[hl<<sec preauth>>]" thingy?
The reference to note 11 above doesn't have a hyperlink. (Not that
I've checked any of them past 11...)
