The CEASE Project Notebook is still online:

It carefully reviewed every aspect before committing to a course of  
action.
I think the project was done so carefully because it represented a  
change from,
SGI as showpiece of Athena, to no support at a time when Athena did a  
LOT of hand-holding of Faculty.

In the project notebook I see reference to a Survey, but I cannot  
find the survey itself.
Perhaps I missed it by scanning too fast.
