Study Offers Young Doctors Strategies To Deal With Discrimination

In July 2013, a man arrived in the emergency room of a California hospital =
seeking treatment for his child. But when the intern on call walked in to s=
ee him, the father looked at her name tag and demanded another physician. A=
s a Palestinian, he didn't want his child treated by a Jewish doctor. The i=
ntern turned to her resident supervisor, Emily Whitgob, who told her collea=
gues about the incident.

The episode helped motivate Whitgob to study how doctors in training and th=
eir institutions should deal with patients' prejudice and to publish a repo=
rt that outlines strategies offered by the professionals she and the other =
authors consulted. The=C3=83=E2=80=9A recommendations, published Wednesday =
in the Association of American Medical Colleges' journal Academic Medicine,=
 call for trainees to focus on their role as doctors by not taking hostile =
comments personally and meeting patients' anxieties with empathy.

Researchers recruited 13 experienced faculty members from the pediatric dep=
artment at Stanford University School of Medicine in California to review a=
 series of scenarios illustrating patient discrimination against a medical =
trainee. Participants shared their reactions to the situation and outlined =
strategies for diffusing the tension in such encounters.

The issue has attracted national attention recently after physician=C3=83=
=E2=80=9A Tamika Cross, a black physician from Houston,=C3=83=E2=80=9A post=
ed on social media that a flight attendant had dismissed her offer to help =
and disputed her credentials when looking for medical aid for a man who nee=
ded treatment mid-flight. Her Facebook post went viral, garnering over 48,5=
00 shares as of Wednesday.

It's not clear how often these episodes occur. But the journal article note=
s that a 2015 survey found that 15 percent of pediatric residents at Stanfo=
rd had experienced or witnessed mistreatment of medical residents by patien=
ts or families. Of those incidents, 67 percent involved discrimination by p=
atients' families. Half the people in the survey said they did not know how=
 to respond to the discrimination and a quarter thought that the hospital w=
as not likely to take any action against the patients' families.

The situations can be complicated by the lack of racial and ethnic diversit=
y in many hospitals. According to the most recent data available from the m=
edical colleges association, minorities excluding Asians made up 9 percent =
of doctors in=C3=83=E2=80=9A 2014.=C3=83=E2=80=9A In the same year, those s=
ame minorities represented=C3=83=E2=80=9A 30 percent=C3=83=E2=80=9A of the =
population.

Whitgob, now a developmental behavioral pediatrics fellow at Stanford, said=
 the results of her research can help create a set of guidelines to give ho=
spitals and medical students a framework to handle patient discrimination i=
n the field.

"I wanted to help create the tools so that they can cope with these events =
in the future," she said.

The experienced doctors in the study noted that if the patient needs immedi=
ate medical attention, providers should ignore any hostile comments and qui=
ckly deliver that care.

When the need is not immediate, the study participants also recommend docto=
rs speak honestly to patients about underlying emotional triggers, which ma=
y perhaps allow a more constructive dialogue that will get to the patient's=
 or family's hostility and may allow both sides to work toward establishing=
 enough trust for care to be given.

Formal training is needed to equip doctors with the skills to engage in the=
se discussions, according to the report. By developing self-awareness and s=
ensitivity, medical personnel can shift the focus away from the discriminat=
ory remarks and emphasize patient care. The group also advised providers to=
 set expectations early in medical training by communicating that discrimin=
ation can happen to anyone.

Many of these concepts should not be new to trainees. As part of their accr=
editation, medical schools already are required to teach students about cul=
turally competent health care.

Rebecca Parker, president-elect of the American College of Emergency Physic=
ians, attended medical schools before cultural competency training was comm=
on and said she was initially surprised when patients objected to working w=
ith her because of her gender.

Today, Parker has made diversity a priority in her organization with a task=
 force to promote diversity in medicine in part through campaigns that chal=
lenge the traditional notions of what a doctor looks like.

"We need to also educate our society about the diversification of our physi=
cian population," Parker said.

Medical training in the classroom alone generally doesn't fill all the need=
s, said Dr. Roderick King, associate professor in the department of public =
health sciences at the University of Miami and CEO of the Florida Institute=
 for Health Innovation. Such training exposes medical students to cultural =
sensitivity, but that may not provide the necessary skills for developing t=
he right approach for these situations. While he said he understands how th=
e fear of insulting a patient could deter a provider from engaging with a p=
atient from a different background, King views cultural competence as a ski=
ll that requires work to perfect.

"The more you practice it," he said, "the more comfortable you get."

Whitgob echoes King's emphasis on real-world training. But a safe learning =
environment is also important to shield trainees from discrimination, she s=
aid. While the experienced doctors admitted facilities can't protect traine=
es from every negative situation, peers and supervisors can alleviate the s=
ting by offering their support. The report recommends institutions set up a=
 chain of command for reporting incidents and affirm their commitment to pr=
otecting its personnel.

In the emergency room incident three years ago, Whitgob almost confronted t=
he patient on her trainee's behalf. Ultimately, the intern worked through t=
he situation, but Whitgob says she's now more aware of the potential for di=
scrimination in the exam room.

"We can't prevent what comes through the door, "Whitcob said. "But we can t=
hink about it on our own time before."

When Christopher Dempsey offered to donate half his liver to a complete str=
anger, he knew it would be a life-changing experience.

What the former marine didn't know was that he would be saving the life of =
the woman who would become his wife.

But less than two years after Mr Dempsey called Heather Krueger to reveal h=
e was a match, the two were married.

The love story began with a conversation overheard in a workplace break roo=
m in Frankfort, a village south of Chicago, Illinois.

Miss Krueger had stage four liver disease, and had been ill for about two y=
ears already when doctors told her she only had a 50% chance of surviving a=
nother two months without a transplant.

Her family were in a race against time to find her a donor - a hugely diffi=
cult task in a country with more than 119,000 people on its transplant wait=
ing lists.

That was where code enforcement officer Mr Dempsey came in.

"I heard a co-worker talking about his cousin who needed a liver transplant=
," Mr Dempsey told the BBC.

"I just thought to myself, I would want someone to help me or my family in =
that situation."

The 38-year-old decided to get himself tested and discovered he was a match=
 for the then Miss Krueger, now 27.

The first time they spoke was the moment he called to tell her he would be =
her donor at the start of February 2015. Later, she would tell him how she =
and her mother had cried tears of joy after receiving the call.

They first met shortly afterwards when Mr Dempsey took Miss Krueger for lun=
ch - and he paid.

In the weeks leading up to the transplant, they began to spend increasing a=
mounts of time together as Mr Dempsey and his motorcycle club threw themsel=
ves into fundraising.

"We were going out looking for donations for a benefit, and I just started =
thinking, she's a really nice girl, she is somebody I would like to get to =
know."

By the time of the surgery at the University Of Illinois Hospital on March =
16, they had already been on a couple of dates, but they only made it offic=
ial afterwards.

"I was really confident everything was going to be okay," said Mr Dempsey. =
"I was still nervous - there is always the chance [something might go wrong=
] - but at no point did I rethink the decision.

"It was just really good knowing she was going to be okay."

The proposal came eight months later, after a horse and carriage ride in Ch=
icago. Then, almost 19 months to the day of the transplant, the healthy cou=
ple became Mr and Mrs Dempsey.

"I think it was definitely more emotional - maybe more so for her and her p=
arents - knowing what we had both been through," said Mr Dempsey.

"I never would have thought in a  years when I agreed to all this that I wo=
uld marry her. It is amazing."

          <p>It's not clear how often these episodes occur. But the journal=
 article notes that a 2015 survey found that 15 percent of pediatric reside=
nts at Stanford had experienced or witnessed mistreatment of medical reside=
nts by patients or families. Of those incidents, 67 percent involved discri=
mination by patients' families. Half the people in the survey said they did=
 not know how to respond to the discrimination and a quarter thought that t=
he hospital was not likely to take any action against the patients' familie=
s.</p>
          <p>The situations can be complicated by the lack of racial and et=
hnic diversity in many hospitals. According to the most recent data availab=
le from the medical colleges association, minorities excluding Asians made =
up 9 percent of doctors in 2014. In the same year, those same minorities re=
presented 30 percent of the population.</p>
          <p>Whitgob, now a developmental behavioral pediatrics fellow at S=
tanford, said the results of her research can help create a set of guidelin=
es to give hospitals and medical students a framework to handle patient dis=
crimination in the field.</p>
          <p>&quot;I wanted to help create the tools so that they can cope =
with these events in the future,&quot; she said.</p>
          <p>The experienced doctors in the study noted that if the patient=
 needs immediate medical attention, providers should ignore any hostile com=
ments and quickly deliver that care.</p>
          <p>When the need is not immediate, the study participants also re=
commend doctors speak honestly to patients about underlying emotional trigg=
ers, which may perhaps allow a more constructive dialogue that will get to =
the patient's or family's hostility and may allow both sides to work toward=
 establishing enough trust for care to be given.</p>
          <p>Formal training is needed to equip doctors with the skills to =
engage in these discussions, according to the report. By developing self-aw=
areness and sensitivity, medical personnel can shift the focus away from th=
e discriminatory remarks and emphasize patient care. The group also advised=
 providers to set expectations early in medical training by communicating t=
hat discrimination can happen to anyone.</p>
          <p>Many of these concepts should not be new to trainees. As part =
of their accreditation, medical schools already are required to teach stude=
nts about culturally competent health care.</p>
          <p>Rebecca Parker, president-elect of the American College of Eme=
rgency Physicians, attended medical schools before cultural competency trai=
ning was common and said she was initially surprised when patients objected=
 to working with her because of her gender.</p>
          <p>Today, Parker has made diversity a priority in her organizatio=
n with a task force to promote diversity in medicine in part through campai=
gns that challenge the traditional notions of what a doctor looks like.</p>
          <p>&quot;We need to also educate our society about the diversific=
ation of our physician population,&quot; Parker said.</p>
          <p>Medical training in the classroom alone generally doesn't fill=
 all the needs, said Dr. Roderick King, associate professor in the departme=
nt of public health sciences at the University of Miami and CEO of the Flor=
ida Institute for Health Innovation. Such training exposes medical students=
 to cultural sensitivity, but that may not provide the necessary skills for=
 developing the right approach for these situations. While he said he under=
stands how the fear of insulting a patient could deter a provider from enga=
ging with a patient from a different background, King views cultural compet=
ence as a skill that requires work to perfect.</p>
          <p>&quot;The more you practice it,&quot; he said, &quot;the more =
comfortable you get.&quot;</p>
          <p>Whitgob echoes King's emphasis on real-world training. But a s=
afe learning environment is also important to shield trainees from discrimi=
nation, she said. While the experienced doctors admitted facilities can't p=
rotect trainees from every negative situation, peers and supervisors can al=
leviate the sting by offering their support. The report recommends institut=
ions set up a chain of command for reporting incidents and affirm their com=
mitment to protecting its personnel.</p>
          <p>In the emergency room incident three years ago, Whitgob almost=
 confronted the patient on her trainee's behalf. Ultimately, the intern wor=
ked through the situation, but Whitgob says she's now more aware of the pot=
ential for discrimination in the exam room.</p>
          <p>&quot;We can't prevent what comes through the door, &quot;Whit=
cob said. &quot;But we can think about it on our own time before.&quot;</p>

           <p>When Christopher Dempsey offered to donate half his liver to =
a complete stranger, he knew it would be a life-changing experience.</p>
           <p>What the former marine didn't know was that he would be savin=
g the life of the woman who would become his wife.</p>
           <p>But less than two years after Mr Dempsey called Heather Krueg=
er to reveal he was a match, the two were married.</p>
           <p>The love story began with a conversation overheard in a workp=
lace break room in Frankfort, a village south of Chicago, Illinois.</p>
           <p>Miss Krueger had stage four liver disease, and had been ill f=
or about two years already when doctors told her she only had a 50% chance =
of surviving another two months without a transplant.</p>
           <p>Her family were in a race against time to find her a donor - =
a hugely difficult task in a country with more than 119,000 people on its t=
ransplant waiting lists.</p>
           <p>That was where code enforcement officer Mr Dempsey came in.</=

 a liver transplant,&quot; Mr Dempsey told the BBC.</p>
           <p>&quot;I just thought to myself, I would want someone to help =
me or my family in that situation.&quot;</p>
           <p>The 38-year-old decided to get himself tested and discovered =
he was a match for the then Miss Krueger, now 27.</p>
           <p>The first time they spoke was the moment he called to tell he=
r he would be her donor at the start of February 2015. Later, she would tel=
l him how she and her mother had cried tears of joy after receiving the cal=
l.</p>
           <p>They first met shortly afterwards when Mr Dempsey took Miss K=
rueger for lunch - and he paid.</p>

           <p>In the weeks leading up to the transplant, they began to spen=
d increasing amounts of time together as Mr Dempsey and his motorcycle club=
 threw themselves into fundraising.</p>
           <p>&quot;We were going out looking for donations for a benefit, =
and I just started thinking, she's a really nice girl, she is somebody I wo=
uld like to get to know.&quot;</p>
           <p>By the time of the surgery at the University Of Illinois Hosp=
ital on March 16, they had already been on a couple of dates, but they only=
 made it official afterwards.</p>
           <p>&quot;I was really confident everything was going to be okay,=
&quot; said Mr Dempsey. &quot;I was still nervous - there is always the cha=
nce [something might go wrong] - but at no point did I rethink the decision=
</p>
           <p>&quot;It was just really good knowing she was going to be oka=

           <p>The proposal came eight months later, after a horse and carri=
age ride in Chicago. Then, almost 19 months to the day of the transplant, t=
he healthy couple became Mr and Mrs Dempsey.</p>
           <p>&quot;I think it was definitely more emotional - maybe more s=
o for her and her parents - knowing what we had both been through,&quot; sa=
id Mr Dempsey.</p>
           <p>&quot;I never would have thought in a years when I agreed to =
all this that I would marry her. It is amazing.&quot;</p>
